Updated: October 2026
In short: Most people regain weight after stopping: in the extension of the STEP 1 trial, participants regained about two thirds of their weight loss within a year. In the SURMOUNT-4 trial, participants who switched to placebo gained 14.0 percent, while those who continued treatment lost a further 5.5 percent. Never stop the injection on your own; plan the step with your doctor. As of: October 2026.
The moment of stopping is often underestimated. The injection dampened hunger and appetite, and that very effect falls away. This article shows what studies show on this and what you can do to hold on to part of what you've achieved. You'll find an overview of the injection itself in the article Weight-Loss Injections: Effects and Side Effects.
What happens after stopping the weight-loss injection?
Appetite and hunger come back, and with them often the weight. The evidence is clear enough that the authors of the STEP 1 extension conclude that continued treatment is needed to maintain the improvements in weight and health. That doesn't mean everyone has to be treated permanently, but that stopping needs a plan.
Important for context: the study figures are group averages. Some participants gained more, others less, and study conditions don't automatically match your daily life. But the figures clearly show the direction and should prompt you to include the end of treatment in your planning early.
The NHS explicitly advises with semaglutide: talk to your doctor before you want to end treatment, and don't end it abruptly. IKK classic (a German health insurer) writes that, with unchanged eating and exercise habits, a large part of the lost weight returns within a year.
What do the studies show on weight regain?
Two studies are particularly revealing.
| Study | Course | Result |
|---|---|---|
| STEP 1 extension (semaglutide 2.4 mg) | 1,961 adults over 68 weeks, after which 327 participants were followed for another year without medication | After stopping, about two thirds of the previously lost weight returned. The net loss compared with baseline by week 120 was 5.6% on average. |
| SURMOUNT-4 (tirzepatide) | 36 weeks of dose escalation with 20.9% weight loss, then 52 weeks of continued treatment or placebo | Continued treatment: a further 5.5% loss. Placebo: 14.0% gain. Total loss after 88 weeks 25.3% versus 9.9%. |
In SURMOUNT-4, 89.5 percent of the group that continued treatment kept at least 80 percent of their weight loss; in the placebo group it was 16.6 percent. That shows how strongly the effect depends on the medication. It also shows that part of the loss remained in both studies.
Why do you regain weight after stopping?
The injection works by reducing appetite, hunger and cravings, as the EMA describes it. When that effect falls away, appetite is back, but the body now has a lower weight and a lower energy requirement. If you eat as before, a calorie surplus results.
That's not a character flaw, but a consequence of the weight loss having been medically supported without habits having become established. If you use the time on the injection to build habits, you start the phase afterwards better. That includes eating rhythm, protein amount, vegetables, exercise and sleep.
How can you limit the yo-yo effect after stopping?
There are no guarantees, but you can improve your starting position. The following points are not study results on stopping, but consequences of the nutrition recommendations for the time on the injection and of general principles.
- Recalculate your calorie needs: With less body weight, your needs fall. The calorie calculator and the article Calorie Needs for Weight Loss help with estimating.
- Keep up protein and strength training: Professional societies cite them as protection for muscle. Less muscle means a lower basal metabolic rate. More on this in the article Nutrition on Weight-Loss Injections.
- Regular meals: Fixed meal times and a protein source in every meal make control easier.
- Fibre and vegetables: These foods fill you up. The guide provides ideas Losing Weight Healthily.
- Keep an eye on your weight: Weigh yourself once a week so you can counteract early without stressing yourself.
What do the first weeks after stopping feel like?
The effect doesn't end abruptly. Because of the long half-life of the active ingredient, the Wegovy prescribing information gives a period of at least two months before a planned pregnancy. That shows that the active ingredient stays in the body for longer. Appetite therefore usually returns step by step, not from one day to the next.
Use this time consciously. For a few weeks, keep a simple hunger and food diary in which you note meals, fullness and mood. That way you see early when cravings occur and what helps then. Plan meals with protein and vegetables ahead so you don't reach for snacks spontaneously out of hunger. And keep moving, even if your weight still looks good.
What role does muscle play in maintaining weight?
Muscle mass uses energy, even at rest. If you lose a lot of muscle while losing weight, energy needs fall more. The US recommendation on nutrition under GLP-1 therapy therefore lists sufficient protein and regular strength training as measures to preserve muscle, with strength training at least three times a week. IKK classic points to the German Society for Nutritional Medicine's warning about sarcopenia.
If you build these habits during treatment, you take them with you into the time afterwards. That's the most important lever you have in your own hands.
When can renewed treatment be a topic?
Whether renewed treatment makes sense after stopping is decided by your doctor based on weight, accompanying conditions and course. The studies show that the effect is tied to the medication. At the same time, part of the loss remains even after stopping, on average 5.6 percent compared with the starting weight in the STEP 1 extension. That too is a gain you can try to hold on to through diet and exercise.
Important: don't start self-treatment with leftover supplies or products from the internet. The Hamburg consumer advice centre (Verbraucherzentrale Hamburg) warns of counterfeits. More on this in the article Peptides for Weight Loss.
How is the injection stopped properly?
Your doctor decides that. The Wegovy prescribing information lists situations in which it must be stopped: with suspected acute pancreatitis, with pregnancy or a planned pregnancy. In adolescents, treatment should be stopped and reassessed if BMI hasn't fallen by at least 5 percent after 12 weeks on 2.4 mg.
For a planned pregnancy, the prescribing information gives deadlines: stop Wegovy at least 2 months beforehand, Mounjaro at least 1 month beforehand. Contraception is recommended during treatment. If you want to stop the injection because of side effects, you should first ask whether a lower dose or a break could help.
For people with type 2 diabetes, stopping is particularly relevant because blood sugar can change. That also belongs under medical supervision.
What speaks against stopping on your own?
If you leave out the injection by feel or change the dose yourself, you risk unnecessary swings in appetite and weight. That also applies when restarting after missed doses: in that case, the prescribing information provides for a medical decision on a lower starting dose. You shouldn't take on dosing information from forums or online sellers.
More on the costs of longer treatment is in the article Wegovy Costs. If treatment goes on for longer, it makes sense to clarify this question early with your doctor.
Pairs well with frank.
