Updated: October 2026
In short: Approved peptide medicines for weight loss in the EU are semaglutide (Wegovy) and tirzepatide (Mounjaro), both prescription-only. What is offered online as a research peptide or retatrutide is not approved and is not subject to any official quality control for identity, content or sterility. The consumer advice centre (Verbraucherzentrale) warns of counterfeits. Get medical advice and only buy medicines with a prescription from a pharmacy. As of: October 2026.
Peptides are a trend on social networks: injections, powders and vials that supposedly work similarly to weight-loss injections, only cheaper. This article separates what is approved, what isn't, and why that makes a difference to your health. How the approved injections work is covered in the article Weight-Loss Injections: Effects and Side Effects.
What are peptides for weight loss?
Peptides are short chains of amino acids. The approved weight-loss injections belong here too: according to the prescribing information, semaglutide is an analogue of human GLP-1, produced by recombinant DNA technology. Online, though, peptides for weight loss often means very different products, namely unapproved substances sold as research products.
The mechanism of action of the approved products is well described. According to the EMA, semaglutide helps regulate appetite by increasing the feeling of fullness and reducing food intake, hunger and cravings. Tirzepatide additionally acts via the GIP receptor. Knowledge about this comes from approval procedures in which quality, effectiveness and safety were checked. With unapproved products, exactly that checking is missing.
That's why the question is worth asking: do you mean an approved medicine with a prescription or a product that is offered without approval? Legally and for your safety, the two are completely different things.
Which peptides for weight loss are approved?
| Active ingredient | Brand name | Approval status in the EU | Prescription |
|---|---|---|---|
| Semaglutide | Wegovy | approved since 6 January 2022 for weight management | yes |
| Tirzepatide | Mounjaro | approved since 15 September 2022 (type 2 diabetes and weight management) | yes |
| Retatrutide | no brand name | not approved in the EU and the US (as of the Verbraucherschutzforum, 7 October 2026) | not available |
Semaglutide and tirzepatide are active ingredients with approval studies and prescribing information. How they differ in detail is explained in the article Wegovy, Ozempic and Mounjaro Compared.
What does Research Use Only mean?
Many sellers write on their products that they are for research purposes only. The Berlin Consumer Protection Forum (Verbraucherschutzforum Berlin) states that this sentence alone does not decide how a product is classified legally. A manufacturer can't get around medicines law by calling a product a research chemical.
Important: the fact that something can be ordered says nothing about whether it's approved as a medicine or whether acquiring it is permitted. According to this account, semaglutide and tirzepatide sold as research versions are also not lawful medicines outside the approved distribution channels.
What risks do research peptides from the internet carry?
The biggest problem is the lack of control. For products offered only as research products, there is, according to the Consumer Protection Forum, no official quality control for identity, active ingredient content or sterility. That means: you don't know what's in the syringe, how much of it, and whether it's sterile.
| Risk | What the sources say |
|---|---|
| Counterfeits | Hamburg consumer advice centre: counterfeits of weight-loss injections keep turning up. |
| Undeclared substances | Active-ingredient cocktails from the internet often contain undeclared substances. Consequences can be a rise in blood pressure and a racing heart, in extreme cases deaths. |
| Lack of quality control | No official testing for identity, content, sterility. |
| Legal consequences | Customs can seize shipments; consequences range from seizure to fines. |
| Dangerous admixtures in weight-loss products | The Rhineland-Palatinate State Investigation Office reports undeclared drugs such as sibutramine in diet pills. |
On top of that, even a genuine product would be risky without medical supervision. The prescribing information lists contraindications and warnings, such as pancreatitis, dehydration or pregnancy. Only someone who knows your history can check those.
Is there evidence for peptides such as retatrutide?
For semaglutide and tirzepatide there are large approval studies. In STEP 1, the average weight change was minus 14.9 percent versus minus 2.4 percent on placebo (68 weeks). In SURMOUNT-1, depending on the dose, it was between minus 15.0 and minus 20.9 percent versus minus 3.1 percent (72 weeks).
For retatrutide, we have no figures from a verified source and therefore give none. What matters anyway: even if studies are under way, that doesn't mean that what is sold online is identical to the study product. The Consumer Protection Forum describes retatrutide as not approved in the EU and the US.
What distinguishes an approved medicine from a research peptide?
| Feature | Approved medicine (Wegovy, Mounjaro) | Research peptide from the internet |
|---|---|---|
| Approval | EU approval by the EMA | no approval as a medicine |
| Prescribing information | Dosage, contraindications, warnings | no verified prescribing information |
| Quality control | officially inspected manufacturing | no official control of identity, content, sterility |
| Supply | Prescription, pharmacy | Online shop, without medical check |
| Studies | Approval studies (STEP 1, SURMOUNT-1) | no evidence that the product matches the study product |
| Monitoring | Wegovy is subject to additional monitoring, side effects should be reported | no monitoring |
The table summarises what follows from the sources. It is not a test of individual sellers.
What should you ask in a doctor's appointment?
If the idea of a weight-loss injection is on your mind, prepare for the conversation. Ask whether your BMI and accompanying conditions meet the approval criteria. Ask about alternatives that are less invasive, about interactions with your medication and about costs, because statutory health insurance doesn't pay here. Ask for a plan for the time after treatment, because after stopping, participants in the STEP 1 extension regained about two thirds of the weight they had lost. More on this in the article Stopping Weight-Loss Injections.
Even if you already have experience with online products, say so openly. That matters for your safety, and nobody needs to be ashamed of it.
What warnings from authorities already exist?
The Rhineland-Palatinate State Investigation Office publishes warnings about weight-loss products with undeclared drugs. For autumn 2025, the page names, among others, Fat Killer Plus, Sliming Bomb and variants of ZeroTermo, and for spring 2026 150G Slim, A1 L-Carnitin Lepidium and Zotreem Dream. The most common finding is sibutramine, an appetite suppressant whose authorisation in Europe was suspended in early 2010 because it can endanger blood pressure and heart, among other things.
That's a different product group from injectable peptides, but it shows the pattern: what looks like a natural product or supplement can contain highly potent substances that aren't on the packaging. If you have a suspicion, you should stop using the product and seek medical advice.
How do you get a weight-loss injection through legitimate channels?
The legitimate route is medical. Approved for weight management are Wegovy and Mounjaro, both with BMI criteria (from 30, or from 27 with an accompanying condition) and as an addition to diet and exercise. They are prescribed after a medical check and dispensed at the pharmacy. On health insurance: statutory insurance doesn't pay for weight loss. Details are in the article Wegovy Costs.
If you're unsure whether an injection is even an option, you can first look at the basics: calorie needs, protein, exercise. The calorie calculator page and the guide Losing Weight Healthily help with that.
What to do if you have symptoms after an online product?
If you've already injected or taken something and get symptoms, for example a racing heart, severe abdominal pain, persistent vomiting or shortness of breath, seek medical help immediately and take the packaging with you. In an emergency, call the ambulance service. Say openly what you took, because only then can the doctor act correctly.
